The Prevent counter-extremism scheme wrongly concluded Southport killer Axel Rudakubana did not pose a serious threat, a government review has found.

Security minister Dan Jarvis MP addressed the House of Commons and said Rudakubana had already discussed the Manchester Arena bombing and stabbing people when Prevent "prematurely" closed his case.
